Parent / Student Forms:

Transportation Change Form - Use this form if there will be a temporary bus change.

Reduced Lunch Form - Use this form to enroll a child into the reduced lunch program.

Internet Usage Form (FERPA) - Use this form to allow your child to access the internet at school .

Student Emergency Contact Info Form - Use this form to update emergency contact info for your child.

Volunteer Background Check Form - Use this form to be able to volunteer at ACS.

Staff Forms:

Field Trip Request Form - Use this form to plan and execute a field trip.

Field Trip Permission Slips - Used to grant children the parental permission to attend a field trip.

Field Trip Bus Request - Used to request a bus for a planned field trip. This form is sent to Buddy Cox.

ACS Cover Sheet - Used for faxing.

ACS Letterhead - Used for forms and letters.
